Individualised Medication Preparation
Individualised Medication Preparation
Nowadays, the ageing population is faced with the need to take medication on a long-term basis. Furthermore, as people get older, the dosages and the number of medicines prescribed become increasingly complex. It is therefore essential to ensure that the right medicine is taken at the right time by the right person. The personalised medication preparation service, available at Correia Rosa pharmacies, involves preparing each patient’s medication for one week, in accordance with the treatment plan prescribed by their doctor. To ensure the quality of the service, the preparation is carried out by a specialist, in accordance with strict hygiene and safety standards. Consequently, the medicines are organised in a single blister pack, separated according to the time of day they are to be taken, thereby ensuring their preservation.
Who is it aimed at?
In other words, those who take more than five medicines, with complex treatment regimens, whether on a long-term basis or on an ad hoc basis.
With little time available to manage their medication properly.
Impaired vision, dementia, memory loss, difficulty understanding medical instructions, etc.
Who are frequently away from home and need to have their medication prepared for specific periods (holidays, business trips, etc.)
What do I need to know?
Blister packs may contain individual solid dosage forms intended for oral administration, such as: capsules, modified-release capsules, tablets, coated tablets, sugar-coated tablets, modified-release tablets and enteric-coated tablets.

- The packaging is practical and easy to handle;
- There is greater control, helping to prevent oversights, contraindications, interactions and duplication of medication when medicines are prescribed by more than one doctor;
- It’s safe; the medication is organised by day of the week and separated into individual doses;
- We ensure adherence to treatment by checking the medicines left in the blister pack that have not yet been taken;

At Correia Rosa pharmacies, once we become responsible for dispensing medication, we manage each patient’s stock of medicines, informing the carer or the patient that they should ask their GP to renew their prescription.
